- Scheduling Appointments

To help provide timely service to our patients, we prefer to schedule all visits. If you are later than 15 minutes for your appointment, out of courtesy to our other patients, we may ask you to reschedule your appointment.
All visits require an appointment except in the case of emergency. Patients who walk in with a non-emergency condition will be fit into the existing schedule as possible, or they will be asked to return later in the day when the schedule allows.
- Forms & Policies

Please allow 3-5 business days for medical forms (for school, sports or camp) to be completed. Forms must be dropped off at the front desk during normal business hours or faxed anytime. Your portion of the form should be filled out completely, including your child’s name and date of birth. If the form requires a doctor’s signature, the child must have had a physical within the last year or it cannot be signed. Please remember to schedule your child’s physical yearly.
Forms for School, Sports or Camp
- Forms must be completely filled in by the parent, including exact name of medications on the bottle and form, ie: pill, liquid, and dosage.

- Telephone Advice

Phone calls about non-urgent matters allow us to answer general questions without actually seeing the child. Messages for the physicians can be left during business hours and will be returned as soon as possible. However, some calls require more time than others, and we ask for your patience. Depending on the nature of the question, we may need to examine the child before diagnosing, treating, or advising on the condition.

You can ask your provider questions via the MyChart app. Message responses may take up to two business days. If this is an emergency, please call 9-1-1.

Please call our main office line and your call will be forwarded to the 24-hour UH Rainbow Advice Center at UH Rainbow Babies & Children's Hospital. One of the nurses will be happy to help you assess your situation and page the doctor if needed. Please save routine questions (mild illness, appointments, prescription refills) for the morning. We encourage you to refer to our website first, when practical, before calling the office.
If an emergency should arise, please call 911. Emergencies are considered to be those conditions that are life-threatening: loss of consciousness, severe bleeding, seizure, etc. In the case of minor emergencies (lacerations, cuts, mild fractures, sprains), please call our office first for instructions.
- Pre-natal Visits/New Babies

We find it important to pick a doctor that you are comfortable with to care for your new baby. Please call to schedule a prenatal visit at no charge. When you are having your new baby, please let the hospital know that we will be caring for him/her so the hospital can send us your newborn's information. We see all newborns within the first week of life.
- Sick Visits

We make every effort to schedule appointments for ill children on the day that you call us. Please call our office ahead of time so that we can minimize the time that you will spend waiting. If you have a medical emergency during office hours, we will do our best to see your child immediately or refer you to the appropriate facility.
If you would like us to see more than one child, please schedule both of them when you make your appointment. Without an appointment, we may not be able to see a sick sibling right away, and you may be asked to wait while the provider sees a scheduled patient who may be waiting. - Well Child Visits/Sports Physical

To keep your children healthy, it is important to bring them in for regular checkups. We recommend that you call our office at least 2 weeks in advance so you may choose the date that best fits your needs. The list below is our recommended schedule for Well Child Visits.
Newborn, 1 month, 2 month, 4 month, 6 month, 9 month, 12 month, 15 month, 18 month, 2 year, yearly after 2 years of age.
- Prescriptions

UH MyChart allows us to e-prescribe any prescriptions at your appointment within seconds. This will allow us to keep better track of all medications you are currently or have been taking, even if prescribed by another office. For prescription refills please try to give as much advance notice as possible; feel free to leave a message for the nurse, we ask that for most refills your child's physical is up-to-date.
Medications that are more closely regulated by the FDA have different requirements for prescribers. We are required by law to see your child regularly to monitor medication use and usefulness.

University Hospitals no longer accepts cash or check as in-person payment for services. You can pay by card (credit, debit, HSA, FSA, etc.), Apple Pay or Google Pay. You can also use UH MyChart to pay your bill online or before your appointment during eCheck-in. We accept checks for payment by mail.

- Insurance Message

We accept most insurance carriers. Please have your card with you at the time of your visit. If we file your claim, you are responsible for your co-pay or deductable at the time of service. If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service.
- Ear Piercing

Why should I choose medical ear piercing?
During our years of practice, we have seen numerous reactions to jewelry and local infections from ear piercing, so we have elected to offer this service to our patients and not-yet patients. In our office your child will receive professional care from our nurse practitioner who has been trained in sterile technique and wound management.

Easy and Affordable
We charge $60 to pierce up to two ears, which also includes the earrings. This is considered a cosmetic procedure, therefore, will not be covered by insurance. The payment will be due at the time of service. We accept patients as young as 6 months for this procedure and we will perform duel piercings at the same time. Ear piercings can be done with well visits as long as the office is notified ahead of time.
