University Hospitals has established a multidisciplinary intervention clinic to help reduce falls and falls related injuries.

- Has the patient fallen in the last six months?
- If yes, did the fall result in an injury?
- Does the patient have a fear of falling?
- Does the patient need assistance with sitting, standing or walking?
- Does the patient need assistance walking in their home?
- Does the patient need assistance in an unfamiliar setting?
- Is the patient using an assistive device?
How to Make a Referral
A referral to the UH Falls Clinic is made in the AEMR.
Simply search for “fall” in the FU/Referral tab.
What happens at the Falls Clinic?
During a one-hour visit, the patient will see multiple specialists from Physical Therapy, ENT, Geriatrics and Pharmacy who will conduct a number of assessments. Customized plans are created per patient and outcome is shared with the primary care provider to ensure continuity of care.
