Going Home – Leaving UH Samaritan

Home healthcare is a popular service, allowing patients to recover in the comfort of their home. Under your physician’s direction, the healthcare professionals at Samaritan Home Care create an individualized treatment plan tailored to your medical needs.

Samaritan Home Care offers these services:

  • Skilled nursing care
  • IV therapy
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Medical social services
  • Certified home health aide
  • PASSPORT Personal care and homemaker services

For optimal continuum of care, staff from our Social Services Department will make arrangements for home healthcare services before you are discharged from the hospital.

In addition, our team of caring professionals will make any necessary arrangements for further medical care once home healthcare services are no longer needed.


How to Pay

Services provided by Samaritan Home Care may be paid, in part or full, by one or more of these:

  • Medicare
  • Medicaid
  • Major medical insurance
  • Health maintenance organizations (HMOS)
  • Worker’s Compensation
  • Other

UH Samaritan Home Care is accredited by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO), certified by the Ohio Department of Health and is an approved PASSPORT provider.
