- Scheduling Appointments

Call the office of your choice from 7 a.m. to 5 p.m. weekdays, 7 a.m. to 12 p.m. Saturday to schedule an appointment during office hours. You can also request a non-urgent appointment by e-mail using our patient portal at UH MyChart. If your child is ill, please call your office for a same day sick appointment.
- Forms & Policies

Please allow 3-5 business days for medical forms (for school, sports or camp) to be completed. Forms must be dropped off at the front desk during normal business hours or faxed anytime. Your portion of the form should be filled out completely, including your child’s name and date of birth. If the form requires a doctor’s signature, the child must have had a physical within the last year or it cannot be signed. Please remember to schedule your child’s physical yearly.

- Forms must be completely filled in by the parent, including exact name of medications on the bottle and form, ie: pill, liquid, and dosage.
- If your child is seen by a specialist, please have the specialist sign the forms.

- Telephone Advice

From 7:00 a.m. to 8:45 a.m. Monday through Saturday, call your primary physician, at any office, with non emergency questions about your child's health. The answering service will take your information, and your physician will call you back. Calls during the day are handled on an individual basis.

You can ask your provider questions via the MyChart app. Message responses may take up to two business days. If this is an emergency, please call 9-1-1.

If you need urgent advice outside of office hours, please call any of our offices and you will be connected to the answering service. The on call physician will return your call. There is a physician on call 24 hours a day, 7 days a week.
If you feel your child is in a life-threatening situation, call 911 or take your child to the nearest emergency room. Notify us, if possible. For less urgent situations, call our office. We may be able to save your child a trip to the emergency room. If your child takes a poison, call the Poison Control Center (800) 222-1222. For emergencies call 9-1-1.
- Pre-natal Visits/New Babies

All of our offices are happily accepting new patients. If you would like to meet with one of our physicians prior to the birth of your baby please call the office of your choice, during regular office hours, to arrange a convenient time. We are glad to meet with you and answer any questions you may have, without any cost to you. Appointment times vary.
- Sick Visits

We make special arrangements to see sick children the same day you call. For your convenience, you can call as early as 7:00 a.m., Monday through Saturday to schedule a same day sick appointment.
Sunday sick visits are available too, at our Garfield Heights location. The answering service can take your call for a Sunday or holiday urgent visit as early as 7:00 a.m. The on call physician will call you back to schedule a time. The office closes after the last sick child is seen, usually half a day, so please call early. There is an additional Sunday/holiday charge assigned that may or may not be covered by your insurance carrier. For questions regarding coverage for this service, please contact your insurance carrier.
- Well Child Visits/Sports Physical

Check ups, camp and sports physicals: please call the office of your choice from 7:00 a.m. to 5:00 p.m. weekdays, 7:00 a.m. to 12:00 p.m. Saturday to schedule an appointment during office hours.
- Prescriptions

UH MyChart allows us to e-prescribe any prescriptions at your appointment within seconds. This will allow us to keep better track of all medications you are currently or have been taking, even if prescribed by another office. For prescription refills please try to give as much advance notice as possible; feel free to leave a message for the nurse, we ask that for most refills your child's physical is up-to-date.
Medications that are more closely regulated by the FDA have different requirements for prescribers. We are required by law to see your child regularly to monitor medication use and usefulness.
- Accepted Forms of Payment

University Hospitals no longer accepts cash or check as in-person payment for services. You can pay by card (credit, debit, HSA, FSA, etc.), Apple Pay or Google Pay. You can also use UH MyChart to pay your bill online or before your appointment during eCheck-in. We accept checks for payment by mail.

- Insurance Message

UH Rainbow Pediatricenter will submit your insurance claims to your primary insurance carrier. As a courtesy, we also coordinate and submit secondary insurance claims. Please be sure to provide our office with your most up to date insurance information. To ensure accurate billing, we ask that you fill out an updated demographic sheet annually. We also ask that you present your insurance card at every visit in order to verify coverage. By law, we are required to collect co-pays at the time of the visit. Co-pays are assigned per your agreement with your insurance carrier. Please contact your insurance carrier directly with any questions regarding co-pays. Several insurance plans will also assign co-insurance in lieu of co-pays. Co-insurance is a percentage of the total bill that the member is responsible for instead of a flat rate. We have a business office (440-720-3519) dedicated to answering any question you may have concerning fees, insurance, or other financial matters.
