- Scheduling Appointments

To help provide timely service to our patients, we prefer to schedule all visits. For an appointment at the Parma office call 440-845-1500. For an appointment at the Brunswick office call 330-225-6374. If you are later than 15 minutes for your appointment, we will try to work you back into the schedule, or perhaps offer an appointment to you with another provider. Out of courtesy to our other patients, we may be forced to ask you to reschedule your appointment.
Patients who walk in with a non-emergency condition will be fit into the existing schedule as best as possible, or they will be asked to return later in the day when the schedule allows.
- Forms & Policies

Please allow 3-5 business days for medical forms (for school, sports or camp) to be completed. Forms must be dropped off at the front desk during normal business hours or faxed anytime. Your portion of the form should be filled out completely, including your child’s name and date of birth. If the form requires a doctor’s signature, the child must have had a physical within the last year or it cannot be signed. Please remember to schedule your child’s physical yearly.
Forms for School, Sports or Camp
- Forms must be completely filled in by the parent, including exact name of medications on the bottle and form, ie: pill, liquid, and dosage.
- If your child is seen by a specialist, please have the specialist sign the forms.

- Telephone Advice

We have nursing staff available daily to assess medical concerns and provide either advice over the phone or to make appointments for medical concerns that are deemed significant enough to warrant a visit.

Our physicians work with the nurses at the UH Rainbow Babies & Children's Hospital after hours advice line to care for any emergencies that occur outside of normal office hours. This service can be accessed by calling our regular office number and listening for instructions. Please note that non - emergency situations, including medication refills and advice regarding chronic medical conditions will not be handled outside of office hours. Please contact the office during regular office hours.
If an emergency should arise, please call 911. Emergencies are considered to be those conditions that are life-threatening: loss of consciousness, severe bleeding, seizure, etc. In the case of minor emergencies (lacerations, cuts, mild fractures, sprains), please call our office first for instructions.
- Pre-natal Visits/New Babies

We offer appointments for expectant parents to visit the office, and ask questions about in-hospital care and newborn follow up. Please call the Parma office at 440-845-1500 or the Brunswick office at 330-225-6374 to schedule these appointments.
- School/Preschool/Daycare

Children who are up to date on their well child care do not need another visit in order for the office to complete school forms. School forms require up to 5 business days to complete, please take this into account when getting your form to the office before its deadline for return. Children who have not had a well visit within the preceding year will need a well child visit prior to completion of any school forms. No exceptions.
- Sick Visits

Dedicated appointments are available for same day sick visits. Please call the office as early as possible to get an appointment for that same day. Walk-in appointments are discouraged. While we will try and accommodate such requests your child will be given the next available, open appointment time and you may be asked to return to the office later in the day if the schedule is full.
- Well Child Visits/Sports Physical

To keep your children healthy, it is important to bring them in for regular checkups. We recommend that you call our office 4 - 6 weeks in advance so you may choose the date that best fits your needs.
- Prescriptions

UH MyChart allows us to e-prescribe any prescriptions at your appointment within seconds. This will allow us to keep better track of all medications you are currently or have been taking, even if prescribed by another office. For prescription refills please try to give as much advance notice as possible; feel free to leave a message for the nurse, we ask that for most refills your child's physical is up-to-date.
Medications that are more closely regulated by the FDA have different requirements for prescribers. We are required by law to see your child regularly to monitor medication use and usefulness.
- Accepted Forms of Payment

University Hospitals no longer accepts cash or check as in-person payment for services. You can pay by card (credit, debit, HSA, FSA, etc.), Apple Pay or Google Pay. You can also use UH MyChart to pay your bill online or before your appointment during eCheck-in. We accept checks for payment by mail.

- Insurance Message

We accept most insurance carriers. Please have your card with you at the time of your visit. If we file your claim, you are responsible for only the co-pay at the time of visit. If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service.
