- Scheduling Appointments

We see patients by appointment only. Please call our office directly to speak with a receptionist, who will arrange an appointment with the doctor of your choice.
- Forms & Policies

Please allow 48 hours for medical forms (for school, sports or camp) to be completed. Forms must be dropped off at the front desk during business hours or faxed anytime. Your portion of the form should be filled out completely, including your child’s name and date of birth. If the form requires a doctor’s signature, the child must have had a physical or check-up, within the last 12 months, or it cannot be signed. We encourage you to schedule your child’s physical/check-up yearly.

- Telephone Advice

Please call our office with non-emergency questions about your child's health. Our answering service will take your information, and your physician or care team will call you back.
- Online Advice

You can ask your provider questions via UH MyChart. Message responses may take up to two business days. If your question is an emergency, please call 9-1-1.
- After Hours/Emergency Calls

Our physicians work with the nurses at UH Rainbow Babies & Children’s Hospital after-hours advice line, to care for any emergencies that occur outside of normal office hours. This service can be accessed by calling our regular office number and listening for instructions. If the after-hours advice nurse feels that our physician on call needs to be involved, one of our physicians will call you at home. Please note that non-emergency situations, such as medication refills, will not be handled outside of normal office hours. Please call the office during regular hours.
- Sick Visits

We make every effort to schedule appointments for ill children on the day that you call us. Please call our office ahead of time so that we can minimize the time that you will spend waiting. If you have an urgent medical issue during office hours, we will do our best to see your child immediately or refer you to the appropriate facility.
- Well Child Visits/Sports Physical

To keep your children healthy, it is important to bring them in for regular checkups. We recommend that you call our office at least 2 weeks in advance so you may choose the date that best fits your needs.
- Prescriptions

Patients and guardians can use UH MyChart to request refills from primary care and specialty providers. Please allow 48 – 72 hours for a prescription to be reviewed unless your child is due to come into the office for an appointment. For most refills, we ask that your child's physical is up to date.
Medications that are more closely regulated by the FDA have different requirements for prescribers. We are required by law to see your child regularly to monitor medication use and usefulness.

You can now quickly and easily pay your provider bill online using our convenient online bill pay tool.
- Accepted Forms of Payment

University Hospitals no longer accepts cash or check as in-person payment for services. You can pay by card (credit, debit, HSA, FSA, etc.), Apple Pay or Google Pay. You can also use UH MyChart to pay your bill online or before your appointment during eCheck-in. We accept checks for payment by mail.
- Pre-natal Visits/New Babies

Our offices are happily accepting new patients. If you would like to meet with one of our physicians prior to the birth of your baby, please call, during regular office hours, to arrange a convenient time to discuss our approach to care and ask questions.
- Fees & Payments

We accept most insurance carriers. Please have your card with you at the time of your visit. If we file your claim, you are responsible for only the co-pay at the time of the visit. If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service.
