Our kidney care navigator services are available for patients with advanced chronic kidney disease. The navigator provides expert patient support and can assist by:
- Communicating with patients to determine needs and concerns
- Helping patients to manage and attend appointments and complete various assignments
- Troubleshooting possible challenges or barriers to care and help to find solutions
In addition, the kidney care navigator can provide patients with education and guidance on:
- Chronic kidney disease/end-stage renal disease
- Hemodialysis evaluation/placement
- Anemia management
- Dialysis initiation
- Kidney transplant evaluation
