An Authorization Form is a form that allows us to double check the services you have authorized for your employees against what we have on your account in the iSystoc EMR. The Authorization Form can be found at Employer Authorization Form. Services will be offered the same way as you are currently used to.

- What is iSYSTOC?

iSystoc is a self-service, online employer tool that provides you employee test results, reports and protocols 24 hours a day, seven days a week. It allows you to check your employees’ work status, results and case summaries online as soon as they are entered into the system, reducing the need to contact University Hospitals Occupational Health directly. iSystoc also allows you to view current and historical employee activity reports and a variety of other standard and customized reports.
