UH Meds PGY1 Community Pharmacy Residency

University Hospitals Meds PGY1 Community-Based Pharmacy residents support patients in reaching their health goals through comprehensive medication management and population health. Residents work in Advanced Primary Care practices alongside an interdisciplinary team to create comprehensive care plans and ensure patient access to medications. Additionally, residents are integral contributors in the transitions of care through the Meds to Beds program, discharge counseling, and medication reconciliation across the University Hospitals system.

Residents with UH Meds PGY1 Community residency have the unique opportunity to work with patients throughout their entire healthcare journey across multiple care settings. UH Meds Ambulatory and Retail pharmacy services continue to grow and innovate delivery of pharmacy services that support patients from the inpatient to the outpatient setting. Residents in this program will serve on the frontlines of patient care, work at the top levels of their degree and license, and will be both leaders and innovators in outpatient pharmacy care.

UH Meds, LLC, offers a range of pharmacy services designed to support a patient’s medication needs in the outpatient setting. These services include the management and delivery of retail medications through UH Meds Ambulatory and Retail Pharmacies, complex specialty medications through UH Meds Specialty Pharmacy, and non-oncology infusion drugs in the home via UH Meds Home Care or in our state-of-the-art facility UH Meds Specialty Care Clinic Infusion Center.
