Advanced Practice Providers
Transition to Practice is a program for newly hired Advanced Practice Providers (APPs) at University Hospitals, Cleveland Medical Center (UH/CMC). This program is designed to provide essential information on common diagnosis and treatment of commonly encountered problems in the acute patient population and specific topics such as quality, safety, documentation, clinical systems and professionalism.
Program goals are accomplished through skill review, didactic and discussion sessions and computer system training. Each clinical area has unit/practice specific orientation and onboarding protocols that allow the newly hired APP to smoothly integrate into the practice team. In addition, the Transition to Practice program offers systems orientation and didactics covering acute care topics commonly encountered in every acute care practice.
Transition to Practice consists of two paid days of didactic sessions and additional EMR systems training.
Clinical area-specific onboarding, along with the Transition to Practice program, allow the newly hired APP to:
- Feel welcomed, comfortable and a part of the organization
- Understand the mission, values and goals of the organization
- Adopt the organization's goals and support its philosophy and vision
- Understand key responsibilities of their position and the impact it has on others
- Provide an overview essential knowledge regarding common problems/diagnoses found in the acute care setting
- How to navigate and use basic clinical systems
- Provide opportunities for networking and ways to develop connections available throughout UH/CMC
The Transition to Practice program incorporates in-person, live didactic lectures on paid time; courses specific to identifying and treating seriously ill patients as well as on-line learning of various acute and intensive care-specific topics.

Additionally, all hospital-based APPs, newly hired as well as established, are encouraged to complete the Fundamental Critical Care Support (FCCS) course, with the goal of preparing the non-intensivists to identify and manage seriously ill patients for the first 24 hours. Skills gained include:
- Prioritizing assessment needs for the seriously ill patient
- Selecting appropriate diagnostic tests
- Identifying and respond to significant changes in the unstable patient
- Recognizing and initiating management of acute life-threatening conditions
- Determining the need for expert consultation and/or until transfer or appropriate critical care consultation can be arranged.
The two-day course is offered free of charge to APPs practicing in the acute setting and contains:
- 17 didactic lectures
- 9 case-based skill stations
- Pre- and post-tests
- Companion textbook
- Certificate of successful course completion recognized by all major certifying bodies for 15 CE/CMEs
The Transition to Practice program is continually being updated to meet the needs of our acute care APPs at here UH/CMC based on evaluations completed by participating APPs as well as APP clinical lead input.
