- What to Bring to my Visit

Please bring your insurance card and a list of current medication lists/doses to each visit.
All co-payments and past due balances are due and payable at the time of service.
- Prescriptions

Requests for refills should be left on your primary physician's nurse line. Be sure to include your name or the child's full name, date of birth, a phone number where you can be reached in case of a question, the name and dosage of the medication and your pharmacy's name and number. You may be asked to schedule a follow up visit before a medication is refilled if deemed necessary by the physician. Refill requests should be made 5 days before the prescription is due to run out. Prescriptions for controlled substances such as ADHD medications, must be picked up at the office. Other prescriptions will be sent by e-mail to your pharmacy, normally by the end of the day.
Our electronic medical record allows us to e-prescribe any prescriptions at your appointment within seconds. This will allow us to keep better track of all medications you are currently or have been taking, even if prescribed by another office. For prescription refills please try to give as much advance notice as possible; feel free to leave a message for the nurse, we ask that for most refills your child's physical is up-to-date.
Patients can use MyChart to request refills from primary care and specialty providers.
- Specialist Referrals

Based on medical necessity, your primary care physician will be able to refer you to a specialist. However, a referral cannot be made without an evaluation by your primary care physician.
If you go to a specialist without a referral, your specialist claim may be denied by your insurance company and the bill will become your responsibility. Due to contractual obligations with your insurance company, we cannot backdate or give you retroactive referrals.

You can access your health information online from anywhere using University Hospitals MyChart. This system allows you to receive lab and radiology results, email a doctor, refill a prescription, request or change appointments and so much more – all from the MyChart app.

Please call 440-892-6424 and your call will be forwarded to the 24-hour UH Rainbow Advice Center at UH Rainbow Babies & Children's Hospital. One of their RN's will be happy to help you assess your situation and page the doctor on-call if needed. - Insurance Information

Always bring your current insurance card with you to all appointments. New or prospective patients should check our list of accepted insurance carriers and confirm that the practice is in-network by contacting their insurance company or by calling our customer service department at 216-383-0100 or 1-800-589-6006.
We participate with Medicare, accept Medicare assignment, and will bill Medicare for you. If you have supplemental insurance please bring that information with you to your appointment. You may be responsible for a portion of your charges such as Medicare deductible and co-insurance.
No Insurance?
UH Westshore Primary Care has a policy in place to help you if you have no insurance or if you have a large balance that you need assistance with. University Hospitals no longer accepts cash or check as in-person payment for services. You can pay by card (credit, debit, HSA, FSA, etc.), Apple Pay or Google Pay. You can also use UH MyChart to pay your bill online or before your appointment during eCheck-in. We accept checks for payment by mail.

UH Westshore Primary Care provides copies of medical records to patients, specialist and primary care physicians at the patients request utilizing the services of Diversified Medical Records. Please ask our team for more information on how to obtain copies of your records from your primary care physician’s office.
