- Scheduling Appointments

Our practice is currently closed to patients eligible for primary or secondary Medicaid, without a provider-to-provider referral request.
Phone calls to schedule appointments will be handled during business hours (Monday through Friday 9:00 a.m. to 4:45 p.m.).
Phone calls to schedule illness appointments for the day may also be made during Telephone Hour directly with a physician (Monday through Friday 8:00 a.m. - 9:00 a.m.).
We reserve the right to reschedule your appointment if you are 15 minutes or more late for a scheduled appointment.
Patients will be seen by appointment only. If you have siblings that need to be evaluated, please make an appointment for each child to ensure that everyone receives an appropriate amount of our time and care. Patients who "walk-in" without a scheduled appointment will be offered the next available time-slot. If you feel your child needs to be seen more urgently, please go to the urgent care or emergency room.
Same day sick appointments are usually available, but requests received late in the day may need to be seen the following morning or referred to the urgent care or emergency room.
- Forms & Policies

Please allow 3-5 business days for medical forms (for school, sports or camp) to be completed. Forms must be dropped off at the front desk during normal business hours or faxed anytime. Your portion of the form should be filled out completely, including your child’s name and date of birth. If the form requires a doctor’s signature, the child must have had a physical within the last year or it cannot be signed. Please remember to schedule your child’s physical yearly.
Forms for School, Sports or Camp
- Forms must be completely filled in by the parent, including exact name of medications on the bottle and form, ie: pill, liquid, and dosage.
- If your child is seen by a specialist, please have the specialist sign the forms.

- Telephone Advice

Messages for the providers and nurse triage line can be left during business hours. However, some calls require more time than others, and we ask for your patience. We typically return calls whenever we have a moment between patients, at lunchtime (12 – 1 p.m.), and at the end of the day. Telephone calls received after 2 p.m. may not be returned until the following day. Depending on the nature of the question, we may need to examine the child before diagnosing, treating, advising on the condition.

After hours urgent calls are handled by our answering service. Please call our main number, 419-626-3821. The answering service will page the physician on call. They will typically return your call within 30 minutes. If you do not receive a call in that time, please call again and confirm your phone number with the answering service. Please save routine questions (mild illness, appointments, prescription refills) for the morning telephone hour.
If an emergency should arise, please call 911. Emergencies are considered to be those conditions that are life-threatening: loss of consciousness, severe bleeding, seizure, etc. In the case of minor emergencies (lacerations, cuts, mild fractures, sprains), please call our office first for instructions.
- Pre-natal Visits/New Babies

Meet and Greet: Our physicians understand how important it is to choose a pediatrician, so we offer a free consultation after hours with one of the pediatricians. Please call the office at 419-626-3821 to set up your consultation.

- Well Child Visits/Sports Physical

“Well Visits” are complete physical examination, sports physicals and preventative/well child checks when your child is not ill and meet the standard of care determined by the American Academy of Pediatrics (AAP). These appointments must be scheduled in advance. For your convenience, our schedule opens one year in advance. Please arrive 10 minutes before your appointment time. We may need to reschedule your appointment if you are 15 minutes or more late for a scheduled well visit appointment. This will allow us to spend ample time with your child and avoid excessive waiting time. The recommended schedule of well visits is as follows: 1 month, 2 month, 4 month, 6 month, 9 month, 12 month, 18 month, 2 years old and then on a yearly basis. Compliance with this schedule is required to maintain current patient status enabling us to continue providing medical care and services to your child.
- Prescriptions

UH MyChart allows us to e-prescribe any prescriptions at your appointment within seconds. This will allow us to keep better track of all medications you are currently or have been taking, even if prescribed by another office. For prescription refills please try to give as much advance notice as possible; feel free to leave a message for the nurse, we ask that for most refills your child's physical is up-to-date.
Medications that are more closely regulated by the FDA have different requirements for prescribers. We are required by law to see your child regularly to monitor medication use and usefulness.
- Accepted Forms of Payment

University Hospitals no longer accepts cash or check as in-person payment for services. You can pay by card (credit, debit, HSA, FSA, etc.), Apple Pay or Google Pay. You can also use UH MyChart to pay your bill online or before your appointment during eCheck-in. We accept checks for payment by mail.

- Insurance Message

We are providers on numerous insurance plans, and our staff cannot be thoroughly familiar with every plan. Please familiarize yourself with your particular insurance plan and know which services are not covered. We ask that co-pays and deductibles be paid at the time of service. Self-pay patients will be asked to pay for services rendered unless other arrangements have been made in advance. Please bring your insurance card to each visit. Failure to provide us with adequate insurance information will require us to bill you directly. Any questions regarding your account can be directed to our billing office at 419-626-3821.
