- Forms & Policies

A grace period of 15 minutes for established patients and 10 minutes for new patients is given after the scheduled appointment time. If you do not arrive within the grace period, you will have to reschedule your appointment.
Valid insurance is due at the time of your appointment. If no valid insurance is on file, you will have to pay for the visit the same day or reschedule. Patients should confirm that the doctor is in their insurance network prior to arriving for their appointment.
Any minor patients need to be accompanied by a parent or guardian.
Paps and STD testing are sent to GENPath Labs for analysis. Patients will need to call if their pap will need to be sent to another lab. Any bloodwork/urine specimens are sent to UH Labs.

You can access your health information online from anywhere using University Hospitals MyChart. This system allows you to receive lab and radiology results, email a doctor, refill a prescription, request or change appointments and so much more – all from the MyChart app.

- Prescriptions

Our electronic medical record allows us to e-prescribe any prescriptions at your appointment within seconds. This will allow us to keep better track of all medications you are currently or have been taking, even if prescribed by another office. For prescription refills please try to give as much advance notice as possible; feel free to leave a message for the nurse. We ask that for most refills your physical is up-to-date.
Medications that are more closely regulated by the FDA have different requirements for prescribers. We are required by law to see you regularly to monitor medication use and usefulness.
Patients can use MyChart to request refills from primary care and specialty providers.
- Accepted Forms of Payment

University Hospitals no longer accepts cash or check as in-person payment for services. You can pay by card (credit, debit, HSA, FSA, etc.), Apple Pay or Google Pay. You can also use UH MyChart to pay your bill online or before your appointment during eCheck-in. We accept checks for payment by mail.
