University Hospitals Breast Health Center Referrals and Physician Resources
Referring to the UH Breast Health Center
UH Breast Health Center professionals promptly respond to hundreds of referring physicians every year. We ask for the following patient information to be available at the time of referral to facilitate the request:

- Current address
- Current phone number(s)
- Insurance information
