How to Enroll in the Program

Check with your insurance provider to confirm you are covered for a tobacco counseling service.

A referral is needed to enroll. Providers can place an order in the EMR, fax a completed referral form to 216-201-5134 or email the form to tobacco.treatment@UHhospitals.org.

Helping Patients Quit and Stay Tobacco-Free

Tobacco use is one of the most preventable causes of death in the United States. It can lead to an increased risk of heart attack, stroke and cancer, as well as vascular and lung disease.

Tobacco use is incredibly addictive, and ending dependence on it can be very difficult. Only a small percentage of people who try to stop using tobacco on their own are successful. Research shows you have a much greater chance of succeeding through a formal tobacco cessation program that combines behavior modification, education and quit aids.
