Diabetes Self-Management Education
The goal of the Diabetes Self-Management Education Program with University Hospitals Medical Practices is to provide knowledge and self-management skills to individuals diagnosed with diabetes and their families/support persons. These understandings and strategies will empower the individual to maintain a physical and psycho-socially healthy lifestyle to their greatest individual capacity. Our mission is to empower individuals with diabetes to effectively manage their diabetes by utilizing their skills that will enable them to live a healthy, active lifestyle.
Type 2 Diabetes
The Diabetes Self-Management Education Program offers comprehensive group educational classes for adults with Type 2 Diabetes. Certified diabetes educators, registered nurses and registered dietitians, who are committed to providing quality patient education, teach the classes. The diabetes classes consists of four group sessions spaced over three to four months. Each session is approximately two hours in length. The classes cover basic topics of diabetes management including: diet, label reading, physical activity, blood glucose monitoring, blood sugar and A1C goals, medications, stress management and behavior changes. Patients who do not have a blood glucose monitor or have an outdated mete will receive a free one at their first session, and they will also be instructed on how to use it.
Type 1 Diabetes, Gestational Diabetes, Insulin Instruction
The Diabetes Self Management Education Program offers comprehensive one-on-one teaching sessions for adults with Type 1 Diabetes, gestational diabetes or who are in need of insulin instruction. These sessions are designed to meet the specific needs of each individual patient.
