University Hospitals is pleased to introduce our multidisciplinary team model of primary care called Advanced Primary Care (APC).

Under this model, primary care providers collaborate with a team of UH healthcare professionals to manage all aspects of your health, including those related to your current and long-term physical, behavioral, functional and financial well-being. And, each member of your APC healthcare team is conveniently co-located with your primary care provider so the services you need are always close-at-hand.

Get Started on the Road to Optimal Health

Your first step is to schedule an appointment with an Advanced Primary Care provider. At your initial consultation, you and your provider will work together to identify your personal goals, current conditions and treatments and any potential barriers to care that may exist. Based on these details, you and your provider will develop a personalized plan to improve, maintain and manage your health.

Your plan will be shared with each member of your APC team so you are all working towards a common goal – your total health and well-being. Team members will then reach out to you to discuss your specific care management needs.

The APC Healthcare Team

Your personalized healthcare team will work together to ensure you achieve and maintain optimal health. Your team will include:

Because Advanced Primary Care is a patient-centered model, you will always be the first and most important member of your healthcare team.

A physician, nurse practitioner or physician assistant who will work with you and other team members to manage all aspects of your health and refer you to specialists as needed.

A licensed, UH pharmacist will be available between doctor appointments to discuss:

Your medications, why they are prescribed and how to take them correctly. He or she will also work with your primary care provider to adjust dosages as needed.
Ways to help manage medication side effects and costs
Your most recent blood sugar and/or blood pressure readings

A registered nurse on your care team will meet with you in person or over the phone to support you in:

Developing a patient centered care plan supporting your personal health goals
Teach you self- management of your care
Address any barriers to your care
Refer you to a multidisciplinary team for additional supports as needed
Keep you connected to your healthcare team
Coordinate your care across the continuum

A mental health professional will be available to discuss any behavioral health concerns you may have, including anxiety and depression.

Your discussion may include:

Your symptoms, history, and treatment and how to optimize your mental health
Brief talk therapy and education to help with your symptoms
Medication management, when needed

Your behavioral health coordinator will communicate with your primary care provider and other mental health specialists to develop a plan-of-care designed specifically for you.
